Four steps at the bedside

Type the UHID. If the patient has come before, their details and earlier records appear. Choose the procedure or the setting (ICU, ward, casualty, OPD), the doctor, and add every relative present with their phone number.
Press Start recording. The consent text or the ready tick-points appear in the family's language. The doctor explains, ticks each point, adds specifics in a note. The timer runs. Press Stop.
One box per person: patient or representative, each attendee, the doctor, the sister who witnessed. Finger or stylus. Each signature is timestamped.
Read the summary, save. The record gets an ID, a hash, and an off-site backup within hours. Print a PDF for the case sheet if you like paper too.
The ICU update at 4 pm was heard by the wife and the son. Both sign. At 9 pm a different doctor explains to the brother who has just arrived. He signs. Tomorrow's dispute has an answer today.
Ready-made points per setting, in Telugu and English: critically ill, risk of death, ventilator may be needed, the next 48 hours are critical, the cost was explained. Tap what was said. Add the specifics.
Recording starts before the doctor speaks and stops after the last question is answered. The file is stored with the record and hashed, so it cannot be swapped or edited later. Play it back from the record page whenever it is needed.
Recording is compulsory for complicated procedures, emergencies, ICU and casualty explanations and refusals. If a device cannot record, the reason is written into the record.
Type a UHID and see every record for that patient: date and time, type, doctor, who was informed, whether audio exists. Print the one-page communication log. Verify any record's integrity with a tap.
