Disputes rarely come from the person who was in the room. They come from a relative who arrives later. A paper consent form cannot answer them: one signature, no time, no proof of what was said, and often the first thing that goes missing.

Who

Every relative or attendant present, by name, relation and phone, each with their own signature. The patient or their representative. The doctor. The sister who witnessed it.

What

The exact consent text and risk list shown, in Telugu and English, frozen into the record. ICU, ward and casualty explanations use ready tick-points so nothing is skipped.

When and how long

Form opened, explanation started and ended, each signature's time, the moment it was saved. Down to the second.

Audio evidence

The conversation itself, recorded on the tablet and stored with the record. Video where the hospital chooses.

Tamper-evident

Every record and recording is sealed with a cryptographic hash. One tap verifies nothing has changed since the day it was signed.

1Procedure consent. OT, minor OT, endoscopy, transfusion. Simple or complicated, with an emergency mode for bedside consent in ICU and casualty.

2Explanation record. ICU updates, ward rounds, casualty risk talks, OPD counselling. Every attendee signs, so nobody can later say they were not there.

3Refusal / LAMA. When advised treatment, admission or referral is refused. Exactly what was advised, the risks explained, and who decided.

Every hospital has its own procedures. Writing a consent template for each one, in English and Telugu, used to take days. Now a doctor types the name and the assistant drafts it in seconds.

Draft with AI

1The draft comes back with the Telugu name, a description a family understands, the usual anaesthesia, and the risks, most common first.

2Bleeding, infection, anaesthesia risk and the rare risk of death are always included, whatever the draft says.

3A doctor reads every line, corrects what is specific to the hospital, and approves. Only then does it reach the consent form, with the doctor's name sealed into every record that uses it.
